How does Cresp 500 mcg Work?

Anemia is defined as low levels of RBCs or hemoglobin that subsequently inhibit oxygen delivery to body tissues. In CKD, the damaged kidneys cannot produce sufficient amounts of erythropoietin, an endogenous glycoprotein hormone, which is normally secreted by renal peritubular cells. Chemotherapy-induced anemia is characterized by the suppression of red blood cell (RBC) production as a result of bone marrow suppression. Both chronic kidney disease and chemotherapy-induced anemia result in diminished proliferation of erythroid progenitors in the bone marrow, slowed differentiation of erythroblasts, and eventually lead to decreased numbers of circulating RBCs in the bloodstream.

To compensate for these changes, the body activates hypoxia-inducible factors (HIFs) to restore erythropoietin production; however, this response is ineffective in states of disease.

Cresp 500 mcg is a synthetic erythropoiesis-stimulating agent (ESA), which mimics the function of natural erythropoietin. Upon binding to the erythropoietin receptor (EPOR) located on erythroid progenitors in the bone marrow, the receptor-ligand interaction activates the JAK2-STAT5 signaling cascade. This pathway promotes erythroblast cell survival, proliferation, and differentiation into mature red blood cells (RBCs), thus improving the RBC cell count. An increase in RBC production results in an increase in hemoglobin levels and improved oxygen levels carried by the RBCs. These changes thus decrease the fatigue and reduce the frequency of RBC transfusions.
