SpeGra is an oral antiretroviral formulation consisting of three active ingredients- Dolutegravir, Emtricitabine, and Tenofovir Alafenamide. It is mainly used for the treatment of Human Immunodeficiency Virus (HIV). These drugs have distinct mechanisms of action, but when combined, they work in unison to control the viral load in the body and slow down disease progression.

Treatment of HIV Infection

HIV is a viral disorder that directly affects the patient's immunity of the patient. These viruses target the CD+ T Lymphocytes and replicate within these cells. For HIV, SpeGra is mainly administered to adults and adolescents (over 12 years). This drug controls the viral load in the body, preventing the disease from progressing. It improves immune function and reduces the risk of transmission to other individuals.

Switching therapy in stable HIV patients

In some cases, SpeGra can be administered to individuals who are already on antiretroviral therapy (ART). In such patients, the viral load is already suppressed. However, older antiretroviral medications often require multiple doses throughout the day and can damage the kidneys and bones. SpeGra generally replaces older regimens as it provides a multi-drug dose in a single dosage. It also has fewer side effects compared to older formulations and ensures better patient compliance.

HIV with risk of resistance (selected cases)

In some patients, the virus can develop resistance to ongoing medications. The resistance can develop due to monotherapies or repeated missed doses. In such cases, SpeGra is administered as it contains a combination of three drugs, which can be consumed by the patient in a single dose. From this combination, Dolutegravir has a high barrier to resistance and is effective for patients with resistant disease.

How does SpeGra Work?

SpeGra is formulated using dolutegravir, emtricitabine, and tenofovir alafenamide. It targets multiple stages of the life cycle of HIV to effectively reduce viral replication.

In an HIV infection, the virus enters the CD4+ T-cell, where it multiplies and spreads to other CD4 + T-cells. In these T cells, the virus converts its RNA to DNA using the reverse transcriptase enzyme. This step is crucial for the viral DNA to replicate and proliferate.

In SpeGra, the drugs emtricitabine and tenofovir alafenamide (TAF) work to inhibit this step by mimicking the nucleotides that are incorporated into the elongating viral DNA chain. As incorrect components are incorporated in the DNA chain, there is premature termination of DNA elongation due to the inability of the virus to create complete genetic material.

TAF, which is considered a prodrug, targets lymphoid cells by enabling the delivery of tenofovir and allowing for its action at lower plasma levels. As a result, systemic toxicity to the kidneys and bones is reduced compared to other previous versions.

The next important step of HIV replication, after the initial replication of HIV's RNA, is the insertion of viral DNA into the cell's DNA (genome) mediated by the enzyme integrase. To prevent this activity, Dolutegravir inhibits the integrase enzyme by blocking the strand transfer process performed by integrase. In this way, as integrase is unable to integrate the viral DNA into the cell, the virus cannot become a permanent part of that cell, and thus cannot spread effectively.

In combination, these products work together to help lower the viral load in the body, and they limit the possibility of infecting new cells and allow for the recovery of the immune system. They also provide effective and long-lasting suppression of the virus by preventing its insertion into host cells.
